Terms of Service

These terms are a template for operators of the Create a Form software. Have them reviewed by legal counsel before relying on them.

1. The service

Create a Form lets you build online forms, collect responses and evaluate them automatically. Features depend on your plan as described on the pricing page.

2. Your account

You are responsible for your account, for keeping API keys secret and for all activity under your account.

3. Acceptable use

• No phishing, credential harvesting or impersonation of other organizations.
• No collection of payment card numbers or government passwords through forms.
• No malware distribution, spam, or illegal content.
• No attempts to bypass rate limits, security controls or plan limits.

We may disable forms or suspend accounts that violate these rules.

4. Your data

You own your forms and the responses you collect. You grant us the rights needed to host and process them to provide the service. You are responsible for having a lawful basis to collect the data you ask for and for informing your respondents.

5. Automatic decisions

Approval workflows apply the criteria you configure. You are responsible for those criteria, for reviewing decisions where the law requires human involvement, and for complying with anti-discrimination rules.

6. Subscriptions

Paid plans are billed in advance through Clerk Billing. You can cancel at any time; access continues until the end of the paid period. Plan limits apply as published.

7. Liability

The service is provided “as is”. To the extent permitted by law, our liability is limited to the fees paid in the last 12 months.
